1. Click here and enter your school email address.
2. You will then be redirected. On the next page, enter the required information and click Continue.
3. Read through the user agreement. Once you have finished, type your full name in the box and click Agree.
4. Click Continue.
5. Select Submit your health forms for free without the upgrade.
